6. Student Request procedure:
- Transcript, Verification certificate and Foundation year certificate: AAO counter -> Fill-in the request form -> Issuance of the Transcript/verification certificate/Foundation Year certificate -> students are called to receive the signed transcript/verification certificate/Foundation Year certificate
- Drop-out: AAO counter -> Fill-in Drop-out request form -> Data entry after approval (Transcript/Foundation Year Certificate/Verification certificate can be issued to the students upon request)
- Suspension: AAO Counter -> Fill-in Suspension request form by stating reason and period and Attach evidence -> Data entry after approval -> Student service unit inform classroom monitor and faculty. Student Service shall remind the students once the due date draws close.
- Transfer:
- Shift: AAO Counter -> Fill-in Shift transfer request form and attach evidence -> Data entry after approval -> Student service unit inform classroom monitor, faculty and report previous score to faculty and update in the student list and attendance book. Students can request for Shift Transfer any time during the semester but if the class size reaches optimum size, the request will be rejected.
- Majors: AAO Counter -> Fill-in Change of Major request form and attach evidence -> Data entry after approval -> Student service inform classroom monitor and faculty, report previous score to faculty and update in the student list and attendance book. Students are allowed to change their major by the end of the 4th week after the semester starts. Change of Major is allowed only in the 2nd semester of 2nd year.
- Study Resume: AAO Counter -> Fill-in study resume request form -> Database unit check student academic records and proceed to H.E Rector for approval -> Data entry after approval -> Student service inform classroom monitor and faculty and update in the student list and attendance book.
- Transfer-in: AAO Counter -> Fill-in transfer-in request form and attach previous Transcript -> student service screen for credit equivalent-> Data entry after approval -> Student service inform classroom monitor and faculty and update in the student list and attendance book. The Year and semester intake is based on the Study pathway policy.
- Transfer-out: AAO Counter -> Fill-in transfer-out request form and attach previous Transcript -> Data entry after approval -> Student service inform classroom monitor and faculty and update in the student list and attendance book. (Transcript/Foundation Year Certificate/Verification certificate can be issued to the students upon request)
- Provisional/Genuine Certificate: AAO Counter -> Fill-in Provisional/Genuine Certificate request form -> Database unit check the Academic completion and other compliance before issuing the Provisional/Genuine Certificate -> students are called to receive the Provisional or Genuine Certificate. Regarding Genuine Certificate can be issued once the certificate is signed by H.E Rector and Board of Directors.
